Plan terms at a glance

Who runs the plan?

Agilio Dental Plans is a trading name of CODEplan Ltd, which administers the collection of subscriptions on our behalf. You can read the full terms and conditions here.

Do I need to book my own appointments?

Yes — it's each patient's responsibility to book. If you don't book or attend your routine appointment on time, we're unable to backdate or refund the missed appointment.

What if my details change?

If you change address, please notify both CODEplan and the practice. If you change bank details, notify CODEplan.

How do I leave the plan?

You may withdraw by giving three months' notice in writing to CODEplan. If you leave within 12 months of receiving a dental appointment, any outstanding amount owed becomes payable to the practice.

Questions about the plans?
